Lemonade data disclosure settlement, who qualifies and what it takes to file

You may qualify if

• ✓You are a living person who lives in the United States.
• ✓Your personal information was compromised in the Lemonade data exposure tied to its online insurance quote platform.
• ✓You received a data exposure notice from Lemonade in April 2025 or June 2025.
• ✓You are not in one of the excluded groups listed in the official settlement notice.

What you need to file

The facts list proof as required, and the how-to shows what that means in practice. The form asks for identifying information, and any documented-loss request needs records behind it: bank statements, credit reports, or letters from tax authorities.

The online form is six steps: open it, enter your identifying details, choose proportional cash or documented losses or both, upload records for any loss claim, submit by 11:59 p.m. ET on September 8, 2026, and save your confirmation. You can also mail a completed form postmarked by that date. If you skip the documented-loss option, there is nothing to upload.

Deadline and what you get

Submit online by 11:59 p.m. Eastern on September 8, 2026, or mail a form postmarked by that date.

The proportional cash payment has no fixed amount. It is a share of what remains in the $10.5 million fund, so it depends on how many valid claims come in. Documented losses pay up to $10,000 against the records you attach.

Lemonade denies wrongdoing and the court has not decided who is right. The Payout app lists this settlement too and points to the same official form.
