If you had a Banner Health patient account (the old MyBanner portal) and logged in at least once between June 1, 2020 and November 22, 2023, you can file for $20 cash plus one year of CyEx Privacy Shield Pro. The form needs no documents.

The case, McCulley v. Banner Health, alleged that Meta Pixel and Google Analytics tags on the portal sent patient information to outside companies. Banner has not admitted wrongdoing. About 1,028,000 patients are covered, and the deadline is September 5, 2026.

You may qualify if

  • You had a Banner Health patient account, formerly called the MyBanner patient portal.
  • You logged into that account on a Banner web property or app at some point between June 1, 2020 and November 22, 2023.
  • You live in the United States and fit the class definition on the settlement site.

Deadline and what you get

File by September 5, 2026. The final approval hearing is September 10, 2026, and payments follow approval.

Every valid claim gets the same thing: $20 in cash and one year of CyEx Privacy Shield Pro, which covers dark web monitoring and password protection. This is a fixed amount, so the number of claims does not change it.

Filing is free on the settlement site, and the administrator decides who is eligible. The Payout app lists this settlement and sends you to the same form.
