What counts as proof of purchase, and what a no-proof claim still asks for

Proof of purchase means any record that shows you bought the thing, when, and what you paid. A store receipt works. So does a card statement with the merchant name, an order confirmation email, a pharmacy printout, or an insurer's explanation-of-benefits statement.
A large share of the settlements on this site ask for no proof at all. Of the 54 cases listed here on August 30, 2026, 22 administrator-run settlements mark proof as not required. That does not mean the form is empty. The administrator uses its own records, a notice ID, or your signed statement instead.
What a no-proof claim still asks for
Almost every no-proof form asks you to sign a statement under penalty of perjury. Disney has you certify your YouTube TV or DirecTV Stream start and end dates that way. Flo Health has you attest that you entered period or pregnancy data between November 1, 2016 and February 28, 2019. RealPage has you list addresses, lease dates and approximate monthly rent, and the administrator can ask for verification later.
Many no-proof forms gate on a code. Comcast Xfinity needs the Class Member ID from the December 2023 breach letter. Penn Medicine needs the unique ID and PIN from the notice. FitOn needs a Login ID and PIN online, though the paper form skips the PIN. If you tossed the letter, the claim site usually has a recovery link or a phone number.
Some no-proof settlements reserve the right to ask. Chantix takes your self-reported prescription history, and the administrator may request pharmacy receipts afterward. Kroger only asks for purchase records when you claim $8,000 or more, or when Kroger's own records did not flag you as a class member.
What a proof-required claim wants to see
The bar is specific. Strides testosterone gel wants a record that names the purchaser, the purchase date, the Strides product or its NDC number, and the out-of-pocket amount. A pharmacy printout, an insurer record, or a pharmacy-benefit-manager statement each cover that. The generic drug settlement accepts pharmacy receipts, invoices and EOB statements going back to May 1, 2009. Your share scales with what you document.
Product cases often want an identifier instead of a receipt. Whirlpool checks your refrigerator's serial number. Hyundai, Kia and Toyota check your VIN through a lookup tool on the settlement site before you fill in anything else. Loss claims, like the $25,000 tier in Americold or the $4,500 total-loss tier in the Hyundai-Kia immobilizer settlement, want the underlying bills: an insurer letter, a police report, bank statements.
If you have no records at all
Check for a flat no-proof tier first. Comcast, Circle K, Labcorp, Equinox, Excel Fitness and Lands' End each pay a fixed or estimated amount ($50, $60 or about $100) to anyone who got the breach notice, with documents required only for the larger loss tier.
Where proof is required and you have none, the administrator denies the claim. Signing an attestation you cannot back up is a false statement on a court-supervised form. Pull your bank's transaction history first. A line item with the retailer's name is often enough.
